Patient Registration
As a new patient to Hillcrest Wellness Center, we'll need to obtain some information about you, including medical history. Please bring this with you to your first appointment. For your first visit, please also be prepared to tell us about any medications that you are currently taking.
Appointments
Patients are seen in our office by appointment. To schedule an appointment, please use our online scheduling option. You may also send us a fax if needed. If you are unable to keep your scheduled appointment, please provide at least 24 hours’ notice so we can offer the time to another patient in need.
